Overview:

Effective cholesterol management, particularly the reduction of low-density lipoprotein cholesterol (LDL-C) is a critical component in the prevention and treatment of cardiovascular disease. This program emphasizes the importance of a team-based approach to cholesterol management including bringing together physicians, pharmacists, dietitians, and other health care professionals to deliver coordinated patient centered care by leveraging the unique expertise of each team member. This cross-functional collaborative model can enhance treatment adherence, optimize therapeutic outcomes, ensure comprehensive patient assessment, and follow up.
